The appointment form records your name, email, optional phone number, treatment selection, preferred appointment time, message, contact preference and consent. Please keep your message brief and avoid sharing sensitive medical details.
Your enquiry is saved privately on this website with access restricted to website administrators. An email copy is also sent to the clinic. The website host and email providers process the information to deliver these services. A short-lived security token is used to limit repeated submissions.
You can withdraw consent or ask the clinic to access, correct or delete your enquiry by email. Withdrawal does not affect processing that already took place.
How long information is kept
Enquiries remain in the private website records until the clinic removes them. Contact the clinic about its retention arrangements or to request deletion. Email copies are managed separately.
